By sending this electronic form to Music Rhapsody, I, as the parent, Agency Representative, or Legal Guardian for the child designated in this form, hereby hold Music Rhapsody harmless for any and all damages, injuries or losses that may be sustained while in the care of Music Rhapsody.

MUSIC RHAPSODY TUITION PAYMENT OPTIONS

Option 1: Annual Discounted Payment
Save 10% on school year tuition! The full discounted yearly tuition, plus processing and materials fees.
Option 2: Payments
Classes run continuously throughout the school year except for holiday breaks. Tuition is prepaid every 4 weeks of classes by debit or credit card only.

If you decide not to continue, you may cancel by filling out a cancellation form from the website, your teacher, or the office. You may also call the office for a cancellation number. Cancellations must be received before the 1st of the month or your card will be charged.

MUSIC RHAPSODY TUITION POLICIES

ENROLLMENT

All registration forms must be completed and submitted to MR with the appropriate payment information, signatures and initials prior to the first class.
A debit or credit card must be on file at MR unless specific arrangements have been approved by the MR office staff prior to enrollment.
Unless paying annually, your credit card will be run on the first (1st) of every month until notification of cancellation is received by the MR office. (Please see CANCELLATION policy below.)
Should you cancel your child's enrollment and wish to continue later in the school year, the processing fee is waived but a $10 re-enrollment fee will be added to your payment to reinstate your child in MR classes.

ATTENDANCE

Tuition paid to MR includes one music class per week except when Music Rhapsody is closed for holidays. A calendar will be emailed to you along with confirmation of payment and enrollment.
There is a $10 switch fee to change the time of your class unless requested by a MR teacher.
Class times are reserved for your enrolled child. If a sibling needs to attend and participate you may arrange for a one class payment with the MR office prior to the class.

CANCELLATION

You may cancel your child's enrollment at Music Rhapsody by notifying a member of the MR staff. You will receive a cancellation number.
Notification of cancellation must be made at least five (5) days prior to the first (1st) of every month.
Refunds and credits for annual tuition will only be considered in the following cases: Relocation, long term illness, injury, or classes closed or cancelled by MR.

REFUNDS

Tuition paid reserves a spot in a class with a music teacher at a specific time therefore no refunds or credits shall be issued for missed classes.
Refunds or credits will not be given due to the failure to cancel enrollment within the five days prior policy as listed above.
Refunds and credits for annual tuition will only be considered in the following cases: Relocation, long term illness, injury, or classes closed or cancelled by MR.

MAKE UPS

Only currently enrolled students may schedule a make up class with a maximum of one (1) per month scheduled at least 24 hours prior to the class.
Unused make ups shall expire six weeks (6) after the date of the missed class. \
Unused make ups may not be carried forward.
Missed make up classes shall be considered forfeited.

CONCERTS

Big Kids Make Music through Ensemble will have an end of the school year concert in which there will be two (2) rehearsals and one performance. The rehearsals and the performance each count as a class.

GENERAL WAIVER (Must be completed!)
